The role emphasizes crisis intervention, documentation, and collaboration with community providers to foster youth resilience and engagement.

Formación

• Experience working with youth in behavioral health settings.
• Ability to develop individual service plans with families.
• Ability to coordinate rehabilitation services per recovery plans.
• Strong documentation and paperwork compliance.

Responsabilidades

• Assist youth with service needs and recovery plans.
• Coordinate rehabilitation services with CORE providers.
• Advocate for resources across agencies.
• Provide crisis support and ongoing assessment.
• Maintain caseload and complete required paperwork.
• Document contact attempts and ISP reviews.
• Conduct monthly caseload reports.
• Perform other duties as assigned.
