Job summary

Rite of Passage in Markleeville, CA is hiring a Case Manager at the Qualifying Freel House to support teen students within our Integrated Care Model. You will develop and monitor case plans, coordinate with guardians and placing agencies, and document progress.

Requirements: Bachelor's degree and at least two years as a Case Manager; Monday–Friday, 8:00am–5:00pm. Pay starts at $26/hour, with full-time benefits including medical insurance eligibility, a 403b match, and PTO.

Qualifications

Be empathetic, patient, and treat others with respect.
Have a desire to work with troubled teens.
Be at least 21 years of age.
Possess a Bachelor's degree.
2 years of experience as a Case Manager.
Be able to pass a criminal background check, drug screen, physical, and TB test.
Be able to pass a search of the child abuse central registry.

Responsibilities

As a Case Manager, you will be responsible for administrating, developing, and facilitating the completion of case plans and treatment programs in accordance with the requests of the placing agency and the needs of each student on their caseload.
Serves as a liaison between the client, parents/legal guardian, and placing agency officials.
Completes required reports and documentation in a timely manner, provides management with required reports, and advises of any problematic situations.
