Job summary

Breakthrough is seeking a dedicated Case Manager for the Back 2 Our Future (B2OF) program in Chicago, IL. You will provide culturally relevant case management, care coordination, and supportive services to youths aged 13-21 under QMHP supervision.

Responsibilities include intake, goal setting, group facilitation, service coordination, and documentation. Travel and flexible hours (including evenings/weekends) may be required.

Qualifications

• Bachelor's degree or equivalent clinical experience in mental health.
• Experience with high-risk populations in Violence Prevention.
• Trauma-informed case management experience.
• Demonstrated accountability in past roles.
• Strong relationship-building and customer service skills.
• Teamwork and initiative in changing environments.
• Sensitivity to cultural and socioeconomic contexts.
• Proficiency with basic Windows software (Word/Excel/Outlook).
• Must have valid driver's license and reliable transportation.

Responsibilities

• Identify and build relationships with at-risk individuals within target community.
• Develop and maintain supportive relationships with participants.
• Coordinate with team and LPHA; manage releases of information as needed.
• Complete intake process; set and monitor safety goals.
• Provide one-on-one sessions and facilitate groups.
• Assist with transportation to supports and services.
• Coordinate internal/external services and resources.
• Maintain participant records in the agency database.
• Engage in care coordination to link youth to community supports.
• Prepare exit plans and coordinate discharge processes.
• Participate in interdisciplinary meetings to discuss progress.
