Job summary

Daybreak is seeking a Care Coordinator to provide individualized, therapeutic-based support to youth and young adults experiencing homelessness in Dayton, OH. You will help YYAs transition from shelter to stable housing, connect them with housing, employment, healthcare, and personal development resources, and create tailored service plans to promote self-sufficiency.

Qualifications

• Requires Bachelor's degree in related field.
• Minimum of two years of program or case management experience.
• Experience working with youth and young adults preferred, especially those experiencing homelessness.

Responsibilities

• Prioritize rapid housing placement with flexible, youth-centered support.
• Provide individualized case management and linkage to resources.
• Foster life skills and independence to support YYA goals.
• Develop and oversee individualized treatment plans with the service team.
• Engage YYAs regularly to build trust and deliver care coordination.
• Conduct home visits and assess progress, at least monthly.
• Assist with schedules, court dates, and official appointments.
• Support exit planning and aftercare for long-term housing stability.
• Act as central point of communication among team and community partners.
• Maintain client records and complete required forms within deadlines.
