Job summary

DuPage County Health Department seeks a full-time Care Coordinator for Pathways to Success, coordinating care for Medicaid-enrolled youth with complex behavioral health needs in DuPage County. You will work with youth, families, and community providers to achieve client goals, maintain weekly contact, develop treatment plans, and participate in planning and reporting.

Requires a bachelor’s degree in Psychology or Social Work and one year of behavioral health experience; Illinois driver’s

Qualifications

• Completion of a bachelor’s degree in Psychology, Social Work, or a related field.
• One year of behavioral health experience; or an equivalent combination of training and experience.

Responsibilities

• Provides care coordination and support services to Pathways-enrolled youth and their families.
• Manages a caseload of clients as defined by the Pathways to Success Program.
• Maintains weekly contact with clients/families.
• Develops child and family teams for clients enrolled in the program and facilitates regular team meetings.
• Provides home-based and community-based services to engage at-risk clients and support client self-management of their illness.
• Develops and maintains client treatment plans.
• Participates in all required HFS planning, training, and reporting activities.
• Works collaboratively with community providers.
• Use of an Electronic Client Record.
• Demonstrates independent judgment and discretion in making position-related decisions.
• Participates in emergency response activities as assigned.
