Job summary

Qualifications

• Bachelor’s degree in a human services field.
• Valid Alabama driver’s license and auto insurance.
• Must become certified as a case manager via DMH-approved training.
• Recovery-oriented approach to providing services.

Responsibilities

• Assist youth/young adults in living independently and pursuing work/education goals.
• Complete strength-based assessments (CANS/SUN-R) and implement service plans.
• Coordinate with families and natural/community allies; liaise with providers.
• Document services clearly in the electronic medical record; ensure continuity of care.
• Provide transportation when necessary and contribute to crisis planning.
