Job summary

Colorado Access seeks a Youth Intensive Care Coordinator to shape our vision and support our mission in the Denver area. You will coordinate high-acuity youth care, develop strengths-based plans, and navigate systems to ensure integrated services.

The role requires a Master’s degree or equivalent experience with 3+ years in clinical care management; bilingual in English/Spanish is a plus; this is a hybrid position in Colorado.

Qualifications

• Master’s degree in social work, human behavioral sciences, or healthcare field and/or clinical license required.
• Combination bachelor’s degree plus 8 years progressive work experience can substitute for master’s degree.
• Three years clinical, care management and/or healthcare experience required; preferred inpatient/outpatient/community health settings.
• Experience with Medicare/Medicaid populations; bilingual in English/Spanish preferred.

Responsibilities

• Coordinate care for high-acuity youth including QRTP, PRTF, CO-SOC services.
• Develop, implement, monitor strengths-based care plans based on CANS/ESA.
• Navigate Child Welfare, CDYS, I/DD, education systems for integrated care.
• Manage in- and out-of-state placements with funding coordination.
• Collaborate with interdisciplinary teams; participate in planning meetings.
• Liaise with providers, families, and stakeholders for service alignment.
• Conduct comprehensive clinical assessments, apply behavioral health knowledge.
• Support CO-SOC care coordination and post-discharge follow-up.
• Educate members/providers on services, benefits, appeals processes; track eligibility; ensure timelines.
