Rockbridge Area Community Services is seeking an Intensive Care Coordinator to provide intensive care coordination to children and adolescents facing emotional and behavioral challenges. The position emphasizes collaboration with families and community partners to develop effective care plans.

Successful candidates will possess a Bachelor's degree and experience working with youth in a clinical role, all while having the opportunity to impact the community positively.

Qualifications

• Minimum two years of experience working with children/adolescents in a clinical or case management role.
• Must obtain CANS certification within 90 days of employment.
• Completion of all Child Mental Health Case Management certification modules within 6 months of employment.

Responsibilities

• Provide intensive care coordination to children and adolescents with emotional and behavioral challenges.
• Develop individualized care plans rooted in family strengths.
• Assist youth in transitioning from residential care.
