Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a CANS Assessment Associate to conduct comprehensive assessments for youth and families across home, school, and community settings.

You will coordinate with families, providers, and partners, maintain compliant documentation, and support timely access to coordinated behavioral health care in southeast Ohio. Strong organizational and communication skills are essential.

Qualifications

• High school diploma or GED is required.
• Current Ohio CANS certification or ability to certify within the first 21 days of employment.
• Experience in community mental health, child welfare, or related human services setting is required.

Responsibilities

• Conduct comprehensive CANS assessments for children, adolescents, and transitional-age youth in virtual, home, school, and community settings.
• Build knowledge of OhioRISE referral pathways and program offerings including 1915C waivers, YFC/Ifast/IHBT, and QRTP requests.
• Maintain accurate records in the electronic health record and CANS portal in line with agency/state requirements.
• Collaborate with clinical staff, families, and community partners to translate assessment findings and next steps.
• Develop collaborative relationships with referral systems and collateral agents involved in CANS/OhioRISE care.
• Use databases and EHR systems for accurate client documentation.
• Maintain HIPAA compliance and mandated reporting requirements.
• Travel within designated service areas; perform other duties as assigned.
