Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a CANS Assessment Associate to conduct comprehensive assessments for children, adolescents, and transitional-age youth across virtual, home, school, and community settings in Southeast Ohio. The role focuses on identifying strengths, needs, and levels of care while coordinating with families, providers, and community partners.

Qualifications

• High school diploma or GED is required.
• Current Ohio CANS certification or ability to certify within the first 21 days of employment.
• 2–3 years of experience in community mental health, child welfare, or related human services setting.

Responsibilities

• Conduct comprehensive CANS assessments for children, adolescents, and transitional-age youth in virtual, home, school, and community settings.
• Build knowledge of OhioRISE referral pathways and program offerings including 1915C waivers and QRTP requests.
• Maintain accurate records in electronic health records and CANS portal per state and agency requirements.
• Collaborate with clinical staff, families, and partners to translate assessment findings and next steps.
• Maintain collaborative relationships with referral systems and collateral agents in the CANS/OHIO RISE care umbrella.
• Utilize EHR and databases for accurate client documentation.
• Maintain HIPAA compliance and ethical guidelines.
• Travel within designated service areas.
• All other duties as assigned.
