Centene seeks a Behavioral Health Case Manager to coordinate care for child and adolescent members with complex mental health needs. This role involves home visits, liaison between families, physicians and providers, and developing care plans to improve outcomes while controlling costs.

Candidates must have an unrestricted behavioral health license and a Master’s degree in Mental Health, Social Work, or Nursing.

Formación

• Requires a Master's degree in Mental Health or Social Work or nursing degree with BH experience.
• Requires an unrestricted behavioral health license for the applicable state.
• Experience coordinating behavioral health services and working with diverse populations.

Responsabilidades

• Evaluates the needs of complex, high-risk members with mental/behavioral health needs and develops care plans.
• Acts as liaison between member/family, physician, and facilities.
• Supports members with mental/behavioral health needs and tracks progress.
• Performs home and site visits to assess needs and coordinate resources.
• Provides education to members and families on care options and benefits.
• Ensures documentation is accurate and compliant with guidelines.
• Complies with policies and standards.
