Job summary

Centene Management Company LLC seeks a qualified Behavioral Health Case Manager to support youth members with complex mental health needs. This role involves field-based engagements, coordinating care plans, and liaison work among families, clinicians, and service providers.

Responsibilities include home visits, resource coordination, and ensuring timely access to services, while adhering to state and federal guidelines.

Qualifications

• Requires a Master's degree in Mental Health or Social Work or Nursing degree.
• 2–4 years of related behavioral health/mental health experience.
• Licensed Behavioral Health Professional or RN per state requirements.

Responsibilities

• Evaluates needs of complex/high-risk members and recommends a care plan.
• Acts as liaison between member/family, physician, and facilities/agencies.
• Supports members with mental/behavioral health needs, including depression, bipolar disorder, PTSD, SUD, etc.
• Performs home or site visits (monthly or more) to assess needs and coordinate resources.
• Education to members and families on preventive care, treatment options, and benefits.
• Coordinates service authorization for services (meals, housing, transportation, etc.).
• Ensures referrals and access to services and documents care management activity in the system.
• Ensures compliance with clinical guidelines and state/federal requirements.
• Provides feedback to leadership to improve quality of care and service delivery.
• Local travel is required for member visits.
