Centene Corp. seeks a Youth Care Manager - Behavioral Health to develop and coordinate holistic care management for children and adolescents with complex mental health needs.

Qualifications

• Master's degree in Mental Health or Social Work or nursing degree.
• 2–4 years of related behavioral health experience.
• 5+ years of Behavioral Health Case Management preferred.
• Licenses: LCSW, LMFT, LMHC, LPC or RN with behavioral health experience.

Responsibilities

• Evaluate needs of high‑risk members with mental/behavioral health needs.
• Act as liaison between member/family, physician and facilities/agencies.
• Support members with major depression, bipolar disorders, and PTSD.
• Perform home/site visits to assess needs and coordinate resources.
• Educate members and families on preventive care and treatment options.
• Coordinate community resources across medical, behavioral and social services.
• Ensure referrals based on member needs and identify providers.
• Maintain documentation in the electronic care management system.
• Provide feedback to leadership to improve quality of care and service delivery.
• Comply with all policies and standards.
