Responsibilities

• Schedule and conduct home visits to assess needs and engagement
• Gather clinical, social, and environmental information for care planning
• Develop person-centered plans focused on strengths and goals
• Maintain relationships with schools, healthcare providers, and partners
• Assist families in accessing behavioral health treatment and resources
• Coordinate care across providers within the CCBHC model
• Advocate for children and families; minimize barriers to services
• Maintain confidential documentation per agency standards
