Job summary

SCAMHC in Alabama is seeking a Children’s Low Intensity Care Coordinator to support children and families across Butler and Crenshaw Counties within a Certified Community Behavioral Health Clinic (CCBHC) framework. You will coordinate services, engage families, and link clients to behavioral health resources with a person-centered approach.

Qualifications

• Bachelor’s Degree required.
• Strong communication, documentation, and organizational skills.
• Ability to work effectively with children, families, schools, and community partners.
• Ability to work independently while collaborating as part of a multidisciplinary team.
• Knowledge of child behavioral health services, family systems, and community resources preferred.
• Professional, flexible, and team-oriented approach to service delivery.

Responsibilities

• Schedule and conduct home visits with children and families to assess needs and support service engagement.
• Gather relevant clinical, social, and environmental information to support individualized care planning.
• Develop and maintain person-centered plans of care focused on the child’s strengths, needs, and treatment goals.
• Establish and maintain collaborative relationships with schools, healthcare providers, social service agencies, and community partners.
• Assist children and families in accessing behavioral health treatment, community resources, educational supports, and other entitled services.
• Support continuity of care and coordination between providers to ensure comprehensive, family-centered service delivery within the CCBHC care model.
• Advocate for children and families while helping reduce barriers to treatment and community support services.
• Maintain accurate, timely, and confidential documentation in compliance with agency standards and CCBHC requirements.
