Jonathan's Place in Garland, TX seeks a Youth Aftercare Program Coordinator to assist survivors in RESET and GRO clients with linkage to aftercare services and re-engage others. You will coordinate transition and discharge plans and conduct community outreach to raise awareness of resources.

The role requires creating discharge plans, 90-day follow-up, and collaboration with partners and staff to tailor care. Travel to schools and partner sites is expected.

Qualifications

• Bachelor's degree or higher in Social Work, Psychology, or Human Services.
• QMHP experience and QMHP Certification a plus.
• 1–2 years of experience with children/youth with behavioral and emotional issues.
• Knowledge of child growth and development.

Responsabilités

• Coordinate aftercare support and services for youth transitioning from programs.
• Develop discharge and after-care plans for youth transitioning from the program.
• Conduct post-discharge follow-up with clients for at least 90 days.
• Lead presentations at area schools and other youth-centered locations.
• Serve as liaison to community partners to disseminate information about the program and agency.
• Collaborate with staff, youth, and community partners in creating individualized transition and discharge plans.
