Job summary

Qualifications

• Must be 21 years of age.
• Open to maintaining a flexible work schedule including evenings.
• Experience working with children with emotional or behavioral health needs.

Responsibilities

• Lead Care Planning Teams for youth services.
• Act as a liaison among individuals and partnering agencies.
• Identify and assist families in accessing services.
