Job summary

CCAP is seeking an FCCP Supervisor to lead care coordination for families in our Rhode Island programs. This non-clinical role emphasizes supervision, case management, wraparound supports, and collaboration with community partners to advance the organization's mission.

Responsibilities include coaching FCCP staff, conducting weekly supervision, and developing treatment and wrap plans while ensuring Medicaid and state standards are met.

Qualifications

• Must hold a master's degree in Social Work or Related Field.
• Must have excellent written and oral communication skills.
• Supervisory experience a plus
• Bilingual Spanish a plus.
• Provide/maintain all required immunizations and/or vaccinations
• Complete all required background checks

Responsibilities

• Provides individual, group, and family wraparound services as needed and appropriate to the needs of the family.
• Provides consultation to professional colleagues, FCCP partners and community groups.
• Develops and maintains good working relationships with area community health providers, schools, legal, welfare and other social/human service and mental health personnel and agencies
• Facilitate and/or participates in interdisciplinary case conferences and /or wraps team meetings both within CCAP and in the community
• Participates in training required by CCAP and other training necessary for the maintenance of professional licensure and/or supervisory requests for growth and improvement.
• Achieves Wrap certification as FSCC and coach/supervisor.
• Provides coaching to FCCP staff, in accordance with high fidelity wrap training and guidelines.
• Completes all written work, Evaluations, reports, progress notes in a timely manner.
• Provides weekly individual supervision to all FCCP (FSCC and FSP) staff.
• Oversees the FCCP records in accordance with CCAP policies and procedures and DCYF standards.
• Assists FCCP staff with the development of treatment/service/wrap plans in accordance with DCYF billing categories.
• Ensures all documents are in accordance with Medicaid and/or state department standards.
• Approves and assigns required documentation in RIFIS.
• Participates in community partnership meetings.
• Other duties as assigned
