Job summary

Comprehensive-Community-Action (CCAP) seeks a dedicated case manager to provide services within the RI Works program, ensuring client compliance and addressing barriers to progress. Responsibilities include assessments, service planning, home visits, and coordination with DHS- and CCAP-supported resources.

The role requires a Bachelor's degree (preferred) and at least 2 years of related experience, with strong case management skills and knowledge of health and behavioral health systems.

Qualifications

• Bachelor's degree; preferred
• Minimum of 2 years related experience
• Must have demonstrated case management skills and experience; emergency intervention methods, family dynamics, health, mental health, and substance abuse systems; and knowledge of community resources
• Valid Driver’s License and proof of liability insurance
• Bilingual in Spanish and/or Creole; preferred

Responsibilities

• To conduct comprehensive assessments (including assessing risk) on assigned clients.
• To develop a service plan with clients in accordance with case management program.
• To provide community and/or home visitation to clients on caseload.
• To provide instruction in adult daily living skills related to barriers for goals.
• To assist with completion of emergency funding, housing, utility assistance, medical and state benefits as needed.
• To maintain current client records and documentation per DHS standards.
• Collaborate with CCAP programs to coordinate services and attend meetings.
• Attend program-related meetings and CCAP public relations activities.
