Job summary

CCAP is seeking an independently licensed clinician for the Family Care Community Partnership (FCCP) program. You will perform initial evaluations, construct clinical formulations, and map out care plans with input from clients and FSCC staff.

The role requires flexibility to provide home-based services and possible evenings/weekends. Qualified candidates hold a master’s in social work or related field and a Rhode Island unrestricted license (LICSW/LMHC/LMFT), with strong communication skills

Qualifications

• Requires master's degree in social work or related field.
• Must hold an unrestricted license to practice in Rhode Island (LICSW, LMFT or LMHC).
• Excellent written and oral communication skills.
• Immunizations/vaccinations required.
• Background checks completed.
• Valid driver's license with proof of insurance.

Responsibilities

• Perform initial diagnostic evaluations and develop clinical formulations.
• Map out and review ongoing care plans with client and FSCC staff input.
• Oversee and complete documentation, evaluations, and progress notes in a timely manner.
• Ensure documents meet Medicaid and state department standards.
• Collaborate with FCCP supervisor to develop treatment services.
• Other duties as assigned.

Education

Master's degree in social work or related field

Unrestricted Rhode Island license (LICSW/LMHC/LMFT)

Bilingual Spanish (a plus)
