Comprehensive-Community-Action in Rhode Island seeks a Family Care Community Partnership (FCCP) Supervisor to manage care coordination for FCCP clients and families. The role provides supervision aligned with CCAP values and oversees data, documentation, and interdisciplinary case conferences.
Fulfilling a full-time schedule, you will supervise staff, coordinate wrap services, and develop treatment plans in line with Medicaid and DCYF standards. Strong communication and leadership are essential.

Supervisors in the Family Care Community Partnership (FCCP) program are responsible for the management of care and case coordination of FCCP clients and their families. Provides comprehensive supervision in keeping with CCAP mission and values of the social work/counseling professions.
