Job summary

PSG is a community-based social services organization in Jefferson County seeking dedicated mental health and social services professionals to join our CCS Service Facilitation program.

The Service Facilitator provides intensive case management for individuals with severe emotional disturbance, coordinating with schools, health and child welfare partners to develop multi-disciplinary treatment plans.

Qualifications

• Bachelor's Degree in Social Work, Psychology, Criminal Justice or related field.
• Minimum one year of experience with severely emotionally disturbed population.
• Reliable daily transportation, valid driver's license, and automobile insurance.

Responsibilities

• Create individualized treatment plans for each client.
• Develop a family support team of formal and informal supports.
• Facilitate team meetings with providers, family members, and family supports to monitor progress.
• Develop a crisis/safety plan for the client and family.
• Advocate for clients in emergency and crisis situations.
• Attend client/family IEP meetings, psychiatry appointments, court proceedings, or other meetings requiring family support or advocacy.
• Safely transport clients as needed.
