Job summary

Integrated Support and Facilitation in Indianapolis, IN is seeking a Wraparound Facilitator to coordinate individualized care plans with youth and families. You will work with a multidisciplinary team to foster stability, growth, and lasting change through strengths-based, family-centered care.

The role requires collaboration, documentation, and the ability to work flexible hours, including some evenings. A valid driver's license and reliable transportation are required to access community

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field required.
• Experience working with at-risk youth, families, or in case management/care coordination.
• Knowledge of the wraparound process and trauma-informed, family-centered care principles.
• Strong communication, active listening, and conflict resolution skills.
• Ability to work flexible hours, including some evenings, to meet family needs.
• Valid driver's license and reliable transportation required.
• Proficiency in documentation and basic computer skills.

• Build trusting, strengths-based relationships with youth and families to identify needs, goals, and supports.
