Canopy Children's Solutions in Mississippi is seeking a Wraparound Facilitator to engage families, build a strength-based team, and develop a holistic single plan of care aligned with high fidelity wraparound practices.

The role requires a bachelor's degree in Social Work or related field, experience with children and families, completion of PCCSS trainings within 90 days of hire, and a flexible schedule including evenings or weekends.

Qualifications

• Bachelor’s degree in Social Work or related field; experience working directly with children and families.
• Must complete PCCSS trainings and certification exam within 90 days of hire.
• Must complete MCC or equivalent Community Support Specialist training within 90 days of hire; flexible schedule.

Responsibilities

• Engage the family to begin high-fidelity Wraparound and educate all team members about the process.
• Gather information for the family story and help develop their family vision for the first 30-day meeting.
• Complete the initial Crisis Management Plan with the family within 24 hours of admission.
• Identify natural/formal/informal supports and have them present at the initial Wraparound Team Meeting.
• Maintain team cohesiveness and foster positive communication between the team and families.
• Facilitate a Wraparound Team Meeting at least every 30 days, or as needed in early stages.
