Job summary

Canopy Children's Solutions is seeking a Wraparound Facilitator in Mississippi to engage families, coordinate a strength-based Wraparound Team, and develop a holistic care plan with high fidelity. This role requires building trusted relationships with families and working across systems to support children with complex mental health needs.

The candidate should complete PCCSS certification within 90 days, have a Bachelor’s in Social Work or related field, and be available for flexible hours

Qualifications

Bachelor’s degree in Social Work or related field.
Must complete PCCSS trainings and certification within 90 days of hire.

Responsibilities

Engage families to begin high-fidelity Wraparound processes.
Gather family information and assist in developing a 30-day Wraparound plan.
Complete initial Crisis Management Plan within 24 hours of admission.
Identify natural/formal supports and include in Wraparound Team.
Maintain team cohesiveness and ensure positive communication with families.
Facilitate Wraparound Team Meetings every 30 days or as needed.

Education

Bachelor’s degree in Social Work or related field

Training and certification as PCCSS per MS DMH requirements within 90 days
