Job summary

Qualifications

• Bachelor’s degree in Human Services, Social Work, Psychology or related field.

Responsibilities

• Conduct monthly and as-needed Child Family Team meetings with families.
• Develop and maintain Plan of Care and Crisis Safety Plan.
• Complete documentation per HIPAA, DMH, DCFS, and county standards.
• Communicate progress or barriers to treatment to referring party.

Aviva Family and Children’s Services Mission StatementCreating safe, supportive spaces, where children, youth and families can heal, build resilience, and experience belonging.Job SummaryUnder the direction of the Wraparound Leadership, the Wraparound Facilitator carries a caseload of children and/or adolescents and their families. The Wraparound Facilitator conducts monthly and as needed meetings with families to assess progress toward their goals and communicates progress or barriers to treatment to the referring party. The Wraparound Facilitator frequently communicates and consults with team members in developing and maintaining a Plan of Care and Crisis Safety plan for families. They are well-versed in and are able to communicate to others the Wraparound philosophy, principles, phases and process.
