Job summary

Aviva Family and Children's Services in Lynwood, CA seeks a Wraparound Facilitator to manage a caseload of children and families, and to develop and monitor care plans following Wraparound. You will lead family meetings, collaborate with the team, and ensure timely documentation.

The role requires strong communication, organizational skills, and the ability to work with diverse populations. A valid CA driver's license and willingness to drive as needed are expected.

Qualifications

  • Bachelor’s degree in Human Services, Social Work, Psychology or related field.
  • 3 years of experience working with high-risk children or youth.
  • Experience in crisis management and Wraparound preferred.

Responsibilities

  • Conduct monthly and as-needed Child Family Team meetings with families.
  • Develop and maintain Plan of Care and Crisis Safety Plan.
  • Complete documentation per HIPAA, DMH, DCFS, and county standards.
  • Communicate progress or barriers to treatment to referring party.
  • Coordinate with team members and outside agencies for services.
  • Adhere to Wraparound philosophy, phases and processes.

Aviva Family and Children’s Services Mission StatementCreating safe, supportive spaces, where children, youth and families can heal, build resilience, and experience belonging.Job SummaryUnder the direction of the Wraparound Leadership, the Wraparound Facilitator carries a caseload of children and/or adolescents and their families. The Wraparound Facilitator conducts monthly and as needed meetings with families to assess progress toward their goals and communicates progress or barriers to treatment to the referring party. The Wraparound Facilitator frequently communicates and consults with team members in developing and maintaining a Plan of Care and Crisis Safety plan for families. They are well-versed in and are able to communicate to others the Wraparound philosophy, principles, phases and process. The Wraparound Facilitator is responsible for completing all required documentation in accordance with agency and County standards.Essential FunctionsResponsive to the families’ identified needsHelps develop family-centered help-giver skillsHelps to achieve professional and family partnershipsAssists family to incorporate informal supports, natural family helpers, and other community resourcesAssists/completes outcome measures with child and familiesResponsible for initial enrollmentsMaintain contact with referring party and communicate family’s participation, progress or barriers to treatmentCollaborate with outside agencies to provide services needed for familiesIn collaboration with team members and families, create and maintain Plan of Care and Crisis Safety Plan for familiesAdheres to a non-judgmental, strength-based model that builds on strengths and hope, while also being realistic and practical.Coordinate, and conduct monthly and as needed Child Family Team meetings with families to assess their needs and progress toward goalsMaintain client chart in accordance with HIPAA, DMH, DCFS, Probation and Los Angeles County standardsComplete and submit all required paperwork, notes and reports including but not limited to:Department of Mental Health progress notesIncident reportsSuspected child abuse reportsMonthly status reportsRespond to crisis and coordinate immediate intervention to stabilize familyConsult with team members and supervisor on a regular basis regarding caseload and appropriate course of action in crisis situations and incidentsLeads the individual Child Family Team by:Following the four phases of Wraparound and all the activities identifiedEnsuring the principles of Wraparound are adhered to by all team membersEnsuring that all the strengths and needs are identified and documentedEnsuring all the identified services are provided in a timely and appropriate mannerBeing the contact point for children, families, service providers and the communityEnsuring the County and/or tribal representatives have adequate opportunities for input and access to the team and planning processAssess the need to request flex funds and communicate need to immediate supervisorRegularly meet with supervisor and/or clinical staffReport any concerns regarding client services and satisfaction to appropriate team memberAttend all program or agency trainings and meetings as requiredDrive personal vehicle as necessarySensitive and responsive to the cultural differences among the agency’s client populationActively participate in the quality improvement projectThe Wraparound Facilitator will take on special projects or additional duties as assigned by the Wraparound Leadership and/or AVP of ICBSCompetency Requirements-Ability to speak, read, write and understand English-Ability to work effectively as a member of the Wraparound team-Ability to successfully work with families of diverse ethnic and cultural backgrounds-Ability to assess a crisis situation and respond in a timely and professional manner-Must have excellent oral and written communication skills-Must have excellent interpersonal skills-Must have strong organizational and planning skills-Knowledge of social services system and community resources-Knowledgeable in Wraparound philosophy, policy, phases and process-Demonstrate skill in operating Microsoft Word, Outlook and Excel-Skilled in relationship building, conflict mediation and facilitation a plus-Bilingual (English/Spanish) preferredEducation and Experience-Bachelor’s degree in Human Services, Social Work, Psychology or related field or 3 years of experience working with high-risk children or youth in either Child Welfare, Probation or Mental Health systems-Experience in crisis management-Experience with Wraparound preferredCertifications/Licenses-Valid CA driver’s licenseAdditional Requirements-Tuberculosis clearance-Department of Justice/Fingerprint clearance-Satisfactory completion of health screening-Proof of legal eligibility to work in the United States-Satisfactory driving recordDelegates Authority ToN/APhysical DemandsWhile performing the duties of a Wraparound Facilitator, the employee is frequently required to walk, drive, and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to site, stand and reach. The employee is rarely required to run, kneel, and lift more than 15 pounds.Work EnvironmentThe work environment is indoors and outdoors. The noise level varies from moderate to loud.
