Job summary

Wellvance in northern Michigan is seeking a full-time Case Manager to provide case management and support services for children and youth up to age 21. You will collaborate with families, schools, and community partners to coordinate comprehensive care and implement person-centered plans while monitoring progress toward goals.

The role requires a BA/BS, completion of MDHHS Wraparound training, and the ability to pass background checks.

Qualifications

• Bachelor’s degree (BA/BS) required.
• Complete MDHHS Wraparound training.
• Must pass criminal history checks and child protection registries.
• Must be supervised by a CHMP who has completed MDHHS training.
• Valid Michigan driver’s license, vehicle insurance, and good driving record.
• Able to travel within the service area.
