Job summary

Wellvance is seeking a dedicated professional to provide case management and support services for children and youth up to age 21. You will collaborate with families, schools, and community partners to ensure comprehensive, person-centered care and monitor progress toward goals.

Documentation must be accurate and align with agency standards. The role requires a Bachelor's Degree, MDHHS Wraparound training, and clearance on background checks.

Qualifications

• Bachelor’s Degree (BA/BS) required.
• Complete MDHHS Wraparound training.
• Criminal history screen, including state and local child protection registries.
• Supervised by a Child Mental Health Professional (CHMP) who has completed MDHHS training.
• Must pass pre-employment background checks (criminal, driving, education, employment).
