Job summary

Wellvance seeks a Home-Based Therapist to provide individual, family, and group therapy for children and youth (birth to 21) in home or school settings, following Medicaid Home-Based guidelines.

The role requires LMSW/LLMSW or LLPC/LPC licensure, with after-hours on-call rotation and ability to travel across Michigan. Strong EMR skills and a valid Michigan driver’s license are essential.

Qualifications

  • Master’s degree in Social Work (MSW) with LMSW or LLMSW licensure, or a master’s degree in Counseling with LPC or LLPC licensure.
  • Ability to participate in after-hours Emergency Services on-call rotation.
  • Valid State of Michigan professional license (full license preferred; limited license accepted).
  • Proficiency in electronic medical record documentation.

Responsibilities

  • Complete intake and annual assessments, IPOS, quarterly reviews, and progress notes in the EHR.
  • Diagnose mental health disorders using the current DSM.
  • Develop and maintain safety plans; ensure copies are in the clinical record.
