Qualifications

• 2-4 years of experience in Case Management and/or Outpatient/Inpatient behavioral health setting preferred.
• Must have a valid driver’s license and own reliable transportation.
• Must be able to pass background checks and provide three professional references.

Responsibilities

• Provide wraparound services to families with up to 15 members/families.
• Coordinate supports, resources, and strategies for each family.
• Lead planning team through engagement, initial plan development and implementation.
