Qualifications

• 2-4 years of experience in Case Management and/or Outpatient/Inpatient behavioral health setting preferred.
• 1-2 years of experience in leading teams preferred.
• Must have a valid driver’s license.

Responsibilities

• Provide wraparound services to families with a limit of up to 15 members/families.
• Coordinate a comprehensive set of supports, resources, and strategies for each family.
• Lead the planning team through engagement and team preparation.
• Monitor the implementation of the plan.
