Job summary

Mid-Columbia Center for Living is seeking a Care Coordinator for the Wraparound program in Children and Family Services. The role requires engaging youth and families, coordinating with community partners, and ensuring fidelity to wraparound processes to empower goals.

The position is on-site in The Dalles and Hood River, OR, with travel 1 day weekly to Hood River. QMHA certification, Oregon CANS completion, and related training are required within defined timelines.

Qualifications

• Must be certified as a QMHA by MHACBO or register as QMHA-R within 14 days of hire.
• Must successfully complete Oregon Wraparound Foundations Training within 6 months of hire.
• Must receive and retain Oregon CANS 0.5-2.0 and 6-20 2.0 certificates within 30 days of hire.

Responsibilities

• Coordinate wraparound services in collaboration with partners and MCCFL Clinical Services Team.
• Initiate parent and youth engagement and support team development with natural and professional supports.
• Develop and implement individualized family plans reflecting youth and family needs and preferences.
