Job summary

Pathways Healthcare seeks a skilled home health nurse to provide comprehensive in-home care across New Hampshire and Maine. You will initiate preventive procedures, administer treatments, assess care needs, and coordinate with physicians to create detailed plans of care for patients.

You will require an active NH nursing license, WCC, and 2+ years of geriatrics experience, with strong bedside manner and CPR/BLS certification. A valid driver’s license and reliable vehicle are essential.

Qualifications

• Excellent bedside manners and communication skills
• Knowledge of healthcare laws and regulations
• Maintain current BLS CPR certification
• Valid driver’s license and own reliable vehicle

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Assess patients and families to determine home care needs.
• Perform thorough physical assessments and collect medical histories.
• Develop and revise plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and teams to create robust care plans.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them.

Position Details:

• Schedule: Monday - Friday
• Salary: $90,000.00 - $115,000.00

Responsibilities:

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Qualifications:

• Must hold an active Nursing license in the State where the position is based.
• Must hold an active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
