Job summary

Pathways Healthcare in Massachusetts seeks a licensed registered nurse to join our Home Health and Hospice team. You will lead preventive and rehabilitative nursing care, administer medications, and perform initial patient assessments in a home care setting.

Qualifications

• Must hold an active Nursing license in the state
• Must hold an active Wound Care Certification (WCC)
• Preferably 2+ years of nursing experience, geriatrics focus
• Nursing degree from an accredited institution
• Excellent bedside manners and communication skills
• Working knowledge of healthcare laws and regulations
• Maintain current BLS CPR certification
• Possess a valid drivers license and own a vehicle

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create Plans of Care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them.

Qualifications:
