Job summary

Responsibilities include creating individualized care plans, educating patients, training staff, and leading skin prevalence studies. Required: Bachelor’s in Nursing, Oregon RN license, BLS, CWON or CWON within 12 months, plus 2 years acute care experience with wound/ostomy exposure.

Qualifications

• Bachelor’s Degree in Nursing is required.
• Oregon RN license is required upon hire.
• BLS certification (American Heart Association) upon hire.
• CWON certification or obtain CWON within 12 months of hire.
• 2 years of recent acute care nursing experience; wound/ostomy experience preferred.

Responsibilities

• Craft personalized wound, ostomy, and skin care plans for hospital to home transitions.
• Educate patients to manage their care with confidence.
• Train and support nursing staff in wound, ostomy, and incontinence care best practices.
• Lead quarterly Skin Prevalence studies and support Skin Care Resource Nurse team.
• Provide inpatient and outpatient services, including clinic follow-up.
