Our Medical Rehabilitation Program offers a meaningful alternative to institutional rehab, helping patients heal at home with dignity.

We are hiring a Wound/Ostomy Nurse - Home Health to join our Westwood, MA team. The role supports Boston and Southshore areas, with a Monday–Friday schedule (8:30–5:00).

Competenze

• Must hold an active RN license in the state where the position is based (Massachusetts).
• Must hold an active WOCN certification.
• Preferably 2+ years nursing experience with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Mansioni

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Conduct initial patient/family assessments to determine home care needs.
• Perform thorough physical assessments and collect medical histories.
• Develop and revise individualized care plans based on patient status.
• Collaborate with physicians, NPs, and inter-disciplinary teams to create Plans of Care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement as part of the care plan.

Job Description:

Qualifications:

Responsibilities:

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
