Job summary

Pathways Healthcare, LLC is seeking a skilled nurse with WOCN certification to lead patient care in Stoneham and surrounding areas. The role supports a home health program with a focus on geriatrics and comprehensive, patient-centered care.

The successful candidate will manage and evolve plans of care in collaboration with physicians and advanced practice providers, ensuring high-quality outcomes and adherence to healthcare regulations.

Qualifications

• Active WOCN certification.
• Active nursing license in the state.
• 2+ years nursing experience focused on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain BLS CPR certification.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust plans of care.
• Respond to inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan.

Job Description:

• Service Area: Stoneham and Surrounding Areas


• Schedule: Monday - Friday (8:30am -5:00pm)


• Job Type: Full Time

Qualifications:

• Hold an active Wound and Ostomy Certification (WOCN).


• Hold an active Nursing license in the State where the position is based.


• Preferably 2+ years of nursing experience, with a focus on geriatrics.


• Nursing degree from an accredited institution.


• Excellent bedside manners and exceptional communication skills.


• Working knowledge of healthcare laws and regulations.


• Maintain current BLS CPR certification.


• Possess a valid driver's license and own a reliable, insured vehicle.
