Job summary

Pathways Healthcare is seeking a skilled registered nurse to join our home health and hospice team in Kingston, PA. This role requires an active nursing license in the state, a WCC and/or OMS certification, and a focus on geriatric care.

You will provide patient-centered home care, administer medications, assess needs, and collaborate with physicians and nurse practitioners to develop plans of care. A valid driver's license and reliable vehicle are required; the schedule is flexible.

Qualifications

• Active nursing license in the state where the position is based.
• WCC and/or OMS certification required.
• Preferably 2+ years nursing experience with geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial patient and family assessments to determine home care needs.
• Perform thorough physical assessments and compile medical histories.
• Develop and revise individualized care plans based on patient status.
• Collaborate with physicians, NPs, and teams to create robust plans of care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement as part of care plans.

• Identify discharge planning needs and proactively implement them as part of the care plan development.
