Job summary

Pathways Healthcare is seeking a full-time nurse in Stoneham, Massachusetts, to provide home health and hospice care. The ideal candidate will have an active nursing license, wound care certification, and 2+ years of experience with geriatrics.

You will collaborate with interdisciplinary teams to develop individualized care plans, while also providing support and counseling to patients and their families. We offer flexible working hours, mileage reimbursement, and comprehensive benefits.

Qualifications

• Must hold an active Nursing license in the State where the position is based.
• Must hold an active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate with physicians and interdisciplinary teams to create Plans of Care.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them.

Job Description:

• Service Area: Stoneham, Melrose, Waltham, Lynn and Surrounding Areas
• Schedule: Monday - Friday (8:30-5:00pm)
• Job Type: Full Time

Qualifications:

• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
