Pathways Healthcare in Westwood, MA is hiring a Wound/Ostomy Nurse for home health. The role covers Boston and Southshore areas with a Monday–Friday schedule (8:30–5:00).

This full-time position emphasizes patient-centered care and AI-assisted documentation to support clinicians. The ideal candidate holds an active nursing license in MA, a WOCN certification, and a nursing degree from an accredited institution, with strong communication and regulatory knowledge.

Qualifications

• Active nursing license in the state of position.
• Active WOCN certification required.
• Prefer 2+ years nursing experience with geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial patient/family assessments to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care.
• Collaborate with physicians, NPs, and teams to create robust plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of care plans.

• Must hold an active Nursing license in the State where the position is based.

• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
