Job summary

Pathways Healthcare is seeking an experienced registered nurse for its Home Health and Hospice programs in Massachusetts. The role is based in Westwood with service coverage for Boston and Southshore areas, operating Monday through Friday during a consistent day-shift schedule.

The ideal candidate will hold an active RN license in MA, a WOCN certification, 2+ years of geriatrics nursing experience, and a solid understanding of healthcare laws. A valid driver's license and vehicle are required.

Qualifications

• Must hold an active Nursing license in the State where the position is based.
• Must hold an active WOCN certification.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

• Salary Range: $90,000 - $115,000 Annual Salary
