Job summary

NYC Health + Hospitals – South Brooklyn Health is seeking a qualified Registered Nurse specializing in wound, ostomy, and continence care. The role focuses on assessment, treatment, and education for patients with complex wounds and ostomies within a multidisciplinary team.

You will provide patient and staff education, develop care plans, and ensure adherence to standards while coordinating with specialists to optimize outcomes.

Qualifications

• Valid NYS RN license and current registration.
• CWOCN/CWON/CWCN/CWS/WCC certification required or in progress.
• BLS certification required.
• Experience in wound care and ostomy care preferred.
• Ability to educate patients and staff.

Responsibilities

• Assess wounds and develop treatment and care plans with the health care team.
• Educate patients, families, and staff on wound prevention and care.
• Collaborate with physicians, nurses, and specialists to coordinate care.
• Document wound status and treatment results and adjust plans as needed.
• Participate in quality improvement initiatives related to wound care.
