Job summary

Pathways Healthcare is seeking a per diem registered nurse in Concord, MA to deliver patient-centered home health care. You will initiate preventive and rehabilitative nursing procedures, assess patients and families, and collaborate with physicians and therapists to develop robust plans of care.

The role requires an active nursing license in the state, WCC certification, and a preference for 2+ years in geriatrics. Valid driver’s license and a reliable vehicle are required.

Qualifications

• Must hold an active Nursing license in the State(s) where the position is based.
• Must hold an active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience with a geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and teams to create robust Plans of Care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them in the care plan.
