Job summary

Pathways Healthcare seeks a per diem registered nurse to deliver in-home nursing and hospice support across Kingston and surrounding areas. You will assess patient needs, administer care, and coordinate with physicians to develop robust care plans.

The role offers flexible scheduling and per visit rates with opportunities for professional growth. Qualified candidates hold an active nursing license, WCC, and a current BLS CPR certification, with preference for 2+ years in geriatrics.

Qualifications

• Active Nursing license in state(s) where position is based.
• Wound Care Certification (WCC) required.
• Preferably 2+ years of nursing experience with geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Current BLS CPR certification.
• Valid driver's license and own reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan.
