Job summary

Pathways Healthcare is seeking a per diem Home Health Nurse to join our physician- and nurse practitioner-led team serving Boston and surrounding areas. You will administer medications, initiate preventive and rehabilitative procedures, and develop plans of care with physicians and NPs.

The role emphasizes compassionate, patient-centered care, collaboration with interdisciplinary teams, and coaching home health aides.

Qualifications

• Active Nursing license in MA (state(s) where based).
• Active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience with geriatrics focus.
• Nursing degree from an accredited institution.
• Current BLS CPR certification.
• Valid driver’s license and insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate with physicians, NPs, and interdisciplinary teams to create robust Plans of Care for each patient.
• Respond to inquiries from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan.

• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
