We are seeking a Wound Nurse (LPN/RN) to join our team at Accela Rehab and Care Center at Somerton. As a member of our nursing team, you will be responsible for providing wound care services to patients with acute and chronic wounds. Your expertise will include identifying wound care needs, developing individualized care plans, and educating patients and families on wound management.
Assess patients' wound care needs and develop individualized care plans
Implement wound care treatments, including debridement, dressing changes, and compression therapy
Monitor patients' wound progress and adjust care plans as needed
Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care
Educate patients and families on wound care techniques and provide emotional support
Document patient care and outcomes in accordance with facility policies
Wound Nurse (LPN/RN) license in the state of Pennsylvania
1 year or more of experience as a Wound Nurse (LPN/RN) or equivalent experience in a related field
Ability to lift, transfer, and reposition patients
Strong communication and interpersonal skills
Familiarity with electronic medical records and wound care software
