Job summary

Care Pavilion in Philadelphia is seeking a dedicated RN Wound Care Nurse to join our interdisciplinary team. You will collaborate with the Director of Nursing, physicians, and unit managers to deliver high-quality wound care to residents in a combined acute and long-term care setting.

The role requires a PA Registered Nurse license, BLS certification, a Wound Care Certification, and at least three years of wound care experience in a long-term care environment.

Qualifications

• Current, active PA RN license.
• BLS Certification.
• Wound Care Certification.
• Three years of wound care experience in long-term care.

Responsibilities

• Assess skin condition of new admissions and readmissions using the standardized assessment tool.
• Use Braden Scales to predict pressure sore risk as needed.
• Implement and administer wound care treatments.
• Document plans of care and update changes in condition.
• Complete initial and weekly skin impairment forms.
• Participate in the Interdisciplinary team and Wound Rounds; contribute to weekly wound report.
