Job summary

Accela Rehab and Care Center at Somerton in Philadelphia, PA seeks a Wound Nurse (LPN/RN) to provide specialized wound care to patients with acute and chronic wounds. You will assess needs, develop care plans, and educate patients and families.

Responsibilities include debridement, dressing changes, compression therapy, tracking progress, collaborating with physicians and therapists, and documenting outcomes in EMR.

Qualifications

• PA nursing license required in the state of Pennsylvania.
• Minimum 1 year experience as a Wound Nurse (LPN/RN) or in a related field.
• Ability to lift, transfer, and reposition patients.
• Familiarity with electronic medical records and wound care software.

Responsibilities

• Assess patients' wound care needs and develop individualized care plans.
• Implement wound care treatments, including debridement, dressing changes, and compression therapy.
• Monitor patients' wound progress and adjust care plans as needed.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.
• Educate patients and families on wound care techniques and provide emotional support.
• Document patient care and outcomes in accordance with facility policies.
