Accela Rehab and Care Center at Somerton in Philadelphia, PA is seeking Licensed Practical Nurses (LPNs) to join our team across all shifts. This full-time role supports rehabilitation and care services under RN supervision, providing medications, assessments, and daily living assistance.

Candidates must hold an active PA LPN license and demonstrate excellent communication, attention to detail, and teamwork in a fast-paced setting.

Qualifications

• Current LPN license in Pennsylvania (PA).

Responsibilities

• Provide direct patient care under RN supervision, including meds and wound care.

As an LPN at our center, you will play a vital role in providing high-quality patient care and assistance in our rehabilitation and care services. Responsibilities:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and other treatments

• Monitor patient vital signs, administer medications, and document patient care in accordance with established medical records

• Collaborate with the interdisciplinary team to develop and implement patient care plans

• Assist with daily living activities, such as bathing, dressing, and grooming

• Maintain a clean and safe environment for patients and staff

• Participate in quality improvement initiatives to ensure optimal patient outcomes

• Demonstrate respect, empathy, and professionalism while interacting with patients, families, and colleagues
