Job summary

OPCO Skilled Management is seeking a Registered Nurse to serve as the coordinator of the center’s wound team, overseeing the skin management program and ensuring quality patient care.

Responsibilities include leading weekly wound rounds, coordinating interdisciplinary care plans, collecting and analyzing pressure ulcer data, and documenting physician orders and preventive interventions. RN/LVN/LPN licensure required with at least one year of nursing management experience preferred.

Qualifications

  • Must hold RN/LVN/LPN licensure in the state
  • One-year prior nursing management experience preferred

Responsibilities

  • Coordinate and lead weekly wound rounds and interdisciplinary care plans.
  • Oversee skin management program and ensure proper documentation.
  • Collect, analyze pressure ulcer data and report findings to staff.
  • Identify center practices and risk minimization measures, including Braden risk scale completion on admission.

Job Type: Full-Time

Objective

A registered licensed nurse designated to act as the coordinator of the center’s wound team to oversee the general application of the skin management program. The following activities are performed as directed or clinically indicated.

Principal Responsibilities
  • Identify and maintain effective methods to gather pressure ulcer and skin alteration related information
  • Coordinate and lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation
  • Collect, analyze and provide pressure ulcer information to nursing staff and other healthcare practitioners
  • Investigate and identify center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions
Qualifications
  • Currently licensed as RN/LVN/LPN in state

One-year prior nursing management experience preferred
